How Can Interim Help?

Interim HealthCare connects patients and caregivers to the CMS GUIDE program by completing the required in-home assessment at no cost. We also deliver respite services for eligible families.

Who is Eligible?

Patients may qualify if they have a dementia diagnosis, are enrolled in Traditional Medicare (Parts A and B), are not enrolled in a Medicare Advantage plan, hospice or PACE, and do not reside in a skilled nursing facility.

Guiding an Improved Dementia Experience (GUIDE) is a program from the Centers for Medicare and Medicaid Services (CMS) that provides enhanced support for patients living with dementia and their caregivers. Through  GUIDE, families gain access to:  Care Navigation, Caregiver Education, Respite Services (up to 72 hours annually), Community Support & Resource Coordination
